Chronic inflammatory demyelinating polyneuropathy in a diabetic patient: deterioration after intravenous immunoglobulins treatment and favorable response to steroid treatment.

چکیده
The authors report the case of a 54-year old type-2 diabetic female patient with a Chronic Inflammatory Demyelinating Polyneuropathy (CIDP). She progressively developed distal hypoesthesia and tetraparesis. She deteriorated after two courses of intravenous immunoglobulins (IVIG) administration and became rapidly wheelchair bound. After one month of steroid treatment, the patient was walking alone. This case raises the question whether IVIG is to be considered as first line treatment for diabetes associated CIDP.

Pure sensory chronic inflammatory polyneuropathy: rapid deterioration after steroid treatment
BACKGROUND Chronic inflammatory demyelinating polyneuropathy (CIDP) as a pure sensory variant is rarely encountered. Therefore the best treatment option is hard to define. CASE PRESENTATIONS We reported two middle-aged patients of Caucasian origin, one female and one male, who over a period of several months presented limbs and gait ataxia. Clinical and neurophysiological examination revealed...
متن کاملO20: NK Cells as Surrogate Marker for Predicting Treatment Efficacy in Chronic Inflammatory Demyelinating Polyneuropathy
Natural Killer (NK) cells are part of our innate immune system with regulatory and effector functions. Different studies suggest that treatment with intravenous immunoglobulins (IVIg) has an immunomodulatory effect on NK cells. IVIg is a first-line treatment for various autoimmune diseases in particular in chronic inflammatory demyelinating polyneuropathy (CIDP). The lack of predictive markers ...
متن کاملPure motor demyelinating neuropathy: deterioration after steroid treatment and improvement with intravenous immunoglobulin.
Within one month of starting oral prednisolone treatment weakness unexpectedly increased in four patients aged 34 to 75 years with purely motor forms of acquired chronic demyelinating neuropathy. By contrast, steroids produced the expected improvement in 11 other patients with symmetric sensorimotor chronic inflammatory demyelinating polyneuropathy. Two of the patients with purely motor demyeli...
متن کاملChronic inflammatory demyelinating polyneuropathy with papillodema and response to immunoglobulins.
Chronic inflammatory demyelinating polyneuropathy is considered as a chronic form of Guillain-Barre syndrome differing from it in response to steroids, course and prognosis. Papillodema although rare is not an uncommon feature. Here we report a case of florid papillodema in a patient of chronic inflammatory demyelinating polyneuropathy, discuss the underlying mechanisms in relation to cerebrosp...
متن کاملOverview of the pathogenesis and treatment of chronic inflammatory demyelinating polyneuropathy with intravenous immunoglobulins
Chronic inflammatory demyelinating polyneuropathy (CIDP) is an acquired heterogeneous disorder of immune origin affecting the peripheral nerves, causing motor weakness and sensory symptoms and signs. The precise pathophysiology of CIDP remains uncertain although B and T cell mechanisms are believed to be implicated. Intravenous immunoglobulins (IVIg) have been shown in a number of trials to be ...
